This position is full-time day shift.

Sentara Norfolk General Hospital is one of just five Level1 trauma centers in Virginia, home to the Nightingale Regional Air Ambulance and the region's first Magnet hospital and nationally ranked heart program at the Sentara Heart Hospital. The hospital sits on the Eastern Virginia Medical Campus which serves as the primary teaching institution. Sentara Norfolk General and the Heart Hospital offers Transplant Services: Heart, Kidney, and Pancreas. We are a Certified Stroke Center and the only Level 3 Epilepsy Center in the area.

This individual provides expertise and support either directly or indirectly by influencing patients/families, nurses/nursing practice, and organization/systems through direct care, leadership, coaching, research, ethical decision making, collaboration, and consultation.
Responsibilities:
The Integrated Care Manager is responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the practice settings of Acute Care, Cardiac Surgical Unit.
Promotes effective utilization and monitoring of health services, collaborates, and communicates with the healthcare team and patient/caregiver to manage care, a safe discharge and to ensure excellent continuity of care.
Meets with multidisciplinary teams to include integrated care manager, nurses, and cardiothoracic surgeons. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in practice settings of Acute Care, Cardiac Surgical Service Line.
Teaches post-operative care such as sternal precautions, lifelong aortic precautions, incisional care, fluid management, diet, weight, activity recommendations, and the value of a cardiac/pulmonary rehab programs.
Schedules follow-up appointments with cardiothoracic surgeons - cardiology, primary care physician, and other specialists as requested by inpatient care team if needed.
