Sentara Norfolk General Hospital in Norfolk, VA is hiring a Team Coordinator MSW to join the Inpatient Case Management Team.

This position is full-time day shift 40 hours per week.

Requirements:

Master's Degree in Social Work required

Licensed LCSW preferred

5 Years Acute Care experience

Previous Supervisory/Leadership experience

The Team Coordinator MSW Takes the leadership for the coordination and provision of medical social work services to patients receiving care in the hospital setting, facilitation transition needs across the continuum of care. Assesses the social, psychological, cultural, environmental, and financial situations, as well as, disposition needs, for each referred patient. Collaborates with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs. Evaluates effectiveness of plans and initiates change as needed. Uses crisis intervention, problem solving model, community organization, and advocacy skills in identifying needs and resources in the hospital and community. Assumes leadership, responsibility and accountability for the team's daily operations including coordination of work, patient satisfaction, quality, and service. Provides mentoring and coaching opportunities for the team, assisting with preceptorship to newly hired social workers and with onboarding of MSW students. Facilitates the efficient and effective workflow of assigned team members. Provides guidance and assistance to members of the multidisciplinary team, physicians, patients and their families in meeting the patients' psychosocial needs.

Five years acute care experience in a hospital, agency or institution providing related health care services preferred (consideration given for related volunteer and field placement experience). Case Management experience strongly preferred. Previous leadership experience preferred.
Certification in Case Management within one year of eligibility. Must meet eligibility requirement upon hire.

Excellent communication and team building skills, demonstrated organization and work management skills, critical and sequential thinking skills. Ability to handle multiple projects and prioritize accordingly. Must be skilled in counseling and group facilitation. Must have a working knowledge of community resources and Virginia Medicaid screening processes. Proficient with case management and transition planning applications. Demonstrates proficient computer literacy.

Qualifications:

MLD - Master's Level Degree: Social Work (Required)

Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI, Licensed Clinical Social Worker (LCSW) - State license - North Carolina Social Work Certification and Licensure Board (NCSWCLB), Licensed Clinical Social Worker (LCSW) - State license - Other/National, Licensed Clinical Social Worker (LCSW) - State license - Virginia Department of Health Professionals (VADHP)
